You also have to select your preferred payment method either cash, credit card, bank deposit, fund transfer or remmittance.
Click Compute Quote to reflect the selected plan.
Select the mode of Payment either monthly, quarterly, semi-annual, annual or spot-cash.
STEP 4:
After choosing the mode of payment, you will be given the option to complete the application.
Simply click Complete Application Now
You will also receive an email from Kaiser so you can have the option to continue the application at another time if you're busy.
Open the email and click the link so you can continue the application.
STEP 5:
Review all the details if all are correct.
If everything is correct then click NEXT.
STEP 6:
Fill-up the online form with the correct details.
At a minimum, you can just fill-up the fields that are required and skip fields that are not mandatory.
STEP 7:
Review your answers again to see if all required fields are filled-up properly.
After review click NEXT
STEP 8:
Fill-up the beneficiary/ies portion.
You have to select a Primary beneficary.
Secondary beneficiary is optional. You can only nominate up to 3 secondary beneficiaries.
STEP 9:
Fill-up your Medical History.
This is the most crucial step of all.
Tip:
If yout think/feel that the answer is "YES", kindly consult with me first for further clarification.

After answering this portion click NEXT.
STEP 11:
If you checked anything in STEP 10, then you must provide the exact details here.
After that, click NEXT
Once you have completed STEP 11, you will receive an email notification indicating that you just completed the application form.
STEP 12:
For your application to be VALID, you must sign online.
Tip:
Open the email on your phone/tablet, click the link and sign using your phone or tablet. Its easier compared to using laptop or desktop.
After that, type your name in the input field then
click I accept the terms of this agreement.
STEP 13:
You will be directed to a payment page.
It is highly recommended that you must accomplish this form first before making any payment.

STEP 14:
Select the payment method from the choices.
Tip:
I personally recommend either Bank Deposit, Remittance or Online Fund Transfer.
For credit or debit card, there will be additional bank charges.

Complete the Order Summary Form then click NEXT

Select your preferred bank.
Please ensure to choose the correct bank.
STEP 17:
You will receive an email with the link where you can upload your proof of payment.
The link is valid for 3-5 days only.
You will continuously receive this email until you upload your payment.
STEP 18
Upload a photo or screen shot of your proof of payment.
Make sure that the image is CLEAR & READABLE.
Once the image appears, click Submit.
Your payment will be under verification and
you will be notified via email once it's approved. Your application will be reviewed by Kaiser Underwriting Department for 2-3 weeks.
You will receive an email once your application has been approved.
